Notes: Polk was a Mecklenburg Declaration signer. At the start of the Revolution he was appointed a subaltern officer of the regiment of the South Carolina Line. He was wounded and confined for 9 months, but otherwise served during the whole struggle. At the time of his death he was the sole surviving field officer of the North Carolina Line. In 1780, he represented Davidson County, now Tennessee, in the General Assembly.
